BENEFIT TERMS & CONDITIONS FOR DISCOVERY “NURTURE@HOME” PARTICIPANTS

  1. General

 

  • These Benefit Terms & Conditions for Discovery “Nurture@Home” Participants (“Benefit T&C’s”) apply to all infant parents (“the Participants”) who are members of the Discovery Health Medical Scheme (registered with the Council for Medical Schemes under registration number 1125) (“the Scheme”) and participate in the Parent Sense Programme as a result of the Scheme’s “Nurture@Home” benefit (“the Benefit”).

 

  • The Parent Sense Programme (“the Programme”) is owned and operated by Sense-IT Ltd (Registration No: 129850), a private limited liability company registered in terms of the laws of Jersey, whose registered office is situated at Floor 4, Liberation House, Castle St, St Helier, Jersey JE1 4HH. The Programme is administered in South Africa by Ayah Sense (Pty) Ltd (Registration No: 2014/255351/07), a private limited liability company registered in terms of the laws of South Africa which has its registered office at 7 Highwick Drive, Kenilworth, Cape Town, South Africa, 7708.

 

  • The Benefit enables Participants to have access to virtual health coaching services provided by healthcare professionals (including paediatric nurses and dieticians) (“the Health Coaches”) specialised in care for babies discharged from neo-natal intensive care units.

 

  • The Benefit is limited for each Participant to 3 (three) consultations of 30 (thirty) minutes each with a Health Coach.

 

  • The Health Coaches are qualified professionals registered with their appropriate healthcare body(ies) and their services are provided independently of Sense-IT Ltd, Ayah Sense (Pty) Ltd, the Scheme and Discovery Health (Pty) Ltd.

 

  • Ayah Sense (Pty) Ltd administers the availability of, and access to, the Health Coaches by the Participants.

 

  • Reliance on the information provided by the Health Coaches is entirely at the risk of the Participant. Sense-IT Ltd, Ayah Sense (Pty) Ltd, the Scheme and Discovery Health (Pty) Ltd are neither responsible, nor do they accept any liability whatsoever, for any loss, damage, harm, injury or death caused to any Participant or infant by a Health Coach.

  1. Consent and Personal Information

 

  • As a member of the Scheme, the Participant consents to the following:

 

  • receiving virtual health coach services including support, education, guidance and behaviour-change coaching services from the Health Coaches;

 

  • undergoing an initial assessment and an Edinburgh Post-Natal Depression Scale assessment via the Parent Sense platform;

 

  • the processing of their and their infant’s personal information in accordance with Sense-IT Ltd’s Privacy Policy found here: https://parentsense.app/privacy-policy/, which processing includes (but is not limited to) audio-visual recordings taken during coaching sessions, the recording of voice notes, calls and other digital interactions, transcriptions of recordings into written notes, including the storage thereof; all of which is aimed at ensuring (amongst other things) clinical continuity, quality assurance and regulatory compliance and is required for audit purposes. Recordings shall not be used for marketing purposes and shall only be accessed by authorised personnel on a “need to know” basis. The processing of such personal information may involve technology-assisted tools (automation or artificial intelligence); and

 

  • the sharing of all their and their infant’s personal information with the Scheme and its administrator, Discovery Health (Pty) Ltd (Registration No: 1997/013480/07).

 

  • For any queries regarding the processing of personal information by Sense-IT Limited or Ayah Sense (Pty) Ltd, please refer to our Privacy Policy (described in clause 2.1.3 above) for the relevant contact details. The Scheme and/or Discovery Health (Pty) Ltd must be contacted in relation to any queries regarding their processing of the personal information of Participants and their infants received from Sense-IT Ltd and/or Ayah Sense (Pty) Ltd.

 

  1. Limited Scope of Health Coach Services

 

  • The services of the Health Coaches (described in clauses 1.3, 1.5, 2.1.1 and 2.1.2 above) shall be provided within their scope of practice and are no substitute for medical advice, diagnosis or treatment.

 

  • Participants are required to use their own reasonable judgment and common sense when acting on the information provided by a Health Coach and to consult a medical practitioner in relation to any medical concern, complaint or condition arising in relation to them or their infant.

 

  • The services of the Health Coaches exclude emergency services and Participants are required to seek their own urgent medical attention from a medical practitioner where required.

 

 

  1. Virtual Health Coach Consultations

 

 

  • All consultations are provided in bundles of 3 (three) sessions of 30 (thirty) minutes each that may be booked and redeemed concurrently or on separate days, at the discretion of the Participant.

 

  • Consultations must be cancelled a minimum of 24 (twenty-four) hours in advance of the appointment.

 

  • If the consultation is not cancelled within 24 (twenty-four) hours, the Participant will forfeit that session but may book the second or third of three sessions.

 

  • If the session is part of a bundle and it is not cancelled/rescheduled within 24 (twenty-four) hours, that session will be deemed to have been redeemed.

 

  • If the consultation is cancelled within 24 (twenty-four) hours of the original appointment, the Participant may rebook an alternative time with the Health Coach based on the latter’s availability.

 

  • If a Participant is late for a consultation, the original start and finish times for that consultation will still apply.

 

  • Ayah Sense (Pty) Ltd shall claim directly from the Scheme the cost per Health Coach consultation offered as part of the Benefit. Any additional sessions required by a Participant outside of the Benefits (three 30min sessions) will be for their personal cost and cannot be claimed against the Scheme.

 

  • Health Coaches may change/reschedule a consultation with 24 (twenty-four) hours’ notice.
